These APTA Health Benefits Marketplace solutions are exclusive APTA member benefits and are open to members who are self-employed and/or own a business.

The process is simple:

1. Fill out the group or individual practitioner request for information form and follow the easy steps.
2. Decisely will contact you with a quote for the health solution.
3. APTA will work with you and Decisely to ensure that membership criteria are met.

To be eligible for this benefit, all physical therapist and physical therapist assistant staff need to have an active APTA membership. Business owners, including individual self-employed practitioners, must also maintain membership with APTA Home Health.
