Excellence in Home Health Leadership Award

Rationale

To recognize the exceptional contribution of an APTA Home Health Academy member in promoting home health physical therapy practice.

Criteria

To be eligible for this award, the nominee must:

  1. Be a current member of the APTA Home Health Academy
  2. Demonstrate the promotion of home health physical therapy through one or more of the following:
  3. Serving as an advocate for the profession of physical therapy within the home health care industry
  4. Serving as an advocate for the home health industry within the field of physical therapy
  5. Demonstrating involvement in activities that encourage innovation in the delivery of home health physical therapy services
  6. Contributing to the development, organization or implementation of a project or program that benefits the consumers of home health physical therapy services
  7. Advocating for physical therapy at the federal or state level

Nomination Process

Any current member of the American Physical Therapy Association may nominate a physical therapist or physical therapist assistant who meets the award criteria. Nomination packets should include:

  1. One or more letters of nomination addressed to the APTA Home Health Academy describing how and why the nominee meets the criteria.
  2. At least one letter of endorsement (support statement) should be included with the nominating letter and address some, if not all, of the following:
    1. Give examples of how this nominee has served as an advocate for the profession of physical therapy within the home health care industry or as an advocate for the home health industry within the field of physical therapy?
    2. List the nominee's involvement in activities that encourage innovation in the delivery of home health physical therapy services.
    3. If applicable, provide examples of the nominee's contributions to a project or program that benefits consumers of home health physical therapy services.
    4. If applicable, provide examples of the nominee's advocacy for physical therapy at the federal or state level.
    5. Anything else you want to add to support why this nominee should be recognized for exceptional contribution in promoting home health physical therapy practice?

Selection Process

The Nominating Committee will review nominations and the chair will present the Committee's recommendation to the APTA Home Health Academy Executive Committee for approval.

Presentation

The award will be presented following the Business Meeting during APTA Combined Sections Meeting, usually held in February of each year. 

The Award will consist of a $50 gift certificate for the APTA Learning Center.
